We understand your condition may be painful and possibly a limiting factor in your life. When scheduling, we will work with you to establish your appointment on the earliest date possible at a time that is convenient for you. Same day appointments are often available. At Waco Foot & Ankle, we do our very best to accommodate you and your unique situation because we want to help get you back to the life you love as quickly as possible.
New Patient Forms
You can choose to call our office at 254-776-6995 to request a link to complete and submit your forms online. You can also use the links below to download the forms to complete and bring into the office during your scheduled appointment.
New Patient Form Pack 2022 – This pack is for New Patients or if you have not been seen in the last thirty-six months. Complete, sign & submit or print and bring with you to your visit.
Form Pack 2 – 2022 – This pack is for existing patients who have been seen in the last 12 months. Complete, sign & submit or print and bring with you to your visit.
Form Pack 3 – This pack is for existing patients who have not been seen in the last 12 months but have been seen in the last 24 – 36 months. Complete, sign & submit or print and bring with you to your visit.
Minor Without Guardian – This form must be completed in order for a minor to be evaluated and treated in our office in the absence of a parent or legal guardian.
Authorization to Release Protected Health Information – Open, complete, sign & submit or print and return to our office.
